I agree that the data I have provided may be processed in accordance with the GDPR for the purpose of providing midwifery services. This includes in particular medical health data (§9 GDPR). If no services have yet been provided, this consent can be revoked at any time. If services have already been provided, the data will only be used to prove the correct processing of the previous activity (e.g.: documentation of the midwifery service, billing of the midwifery service). The withdrawal of consent does not affect the lawfulness of the data processed on the basis of the consent until the withdrawal.
Your data will be irrevocably deleted after expiry of the statutory retention obligations for documentation and invoicing (30 years for home births, 10 years in all other cases). I will not pass on your data to third parties without your consent - with the exception of legal obligations such as billing.
I also agree that midwife Laetitia C. Hecht may forward personal data to the tax office “Busch & Stühmeier” in Bad Oeynhausen for tax consultancy purposes. Like the midwife, the tax office observes the statutory duty of confidentiality in accordance with §203 StGB.
The retention obligation under tax law applies before a GDPR deletion claim. Earlier deletion of invoices is therefore excluded.
I process the data in accordance with the current data protection regulations.
You have the following rights in relation to your personal data
You can contact me at any time to assert your rights.
If you believe that the processing of your data violates data protection law or your data protection claims have otherwise been violated in any way, you can complain to the competent supervisory authority.

Consumers are entitled to a right of withdrawal under the following conditions: A consumer is any natural person who concludes a legal transaction for purposes that cannot be predominantly attributed to either their commercial or independent professional activity. The midwife/midwifery practice points out the following to the participant: You have the right to cancel this contract within 14 days without giving reasons. The cancellation period is 14 days from the day the contract is concluded. In order to exercise your right of withdrawal, you must inform the midwife of your decision to withdraw from this contract by means of a clear statement (e.g. a letter sent by post or by email). In order to meet the cancellation period, it is sufficient that you send the notification of your exercise of the right of cancellation before the cancellation period expires.
Consequences of revocation
The midwife/midwifery practice must repay all payments received from the participant immediately, but at the latest within 14 days from the day on which notification of the revocation was received. If the participant has requested that the service begin during the cancellation period, she must pay the midwifery practice an appropriate amount that corresponds to the proportion of the service used up to that point.
